This document provides guidelines for activities sponsored by EdTA and for activities sponsored by Chapters and Troupes at various venues both on and off school campuses in order to foster a safe, educational and enjoyable environment for all members and participants. Except as noted herein, EdTA may, on their own, adopt policies that are stricter or adopt specific policies provided by sponsoring school districts other than the policies listed in this document. Chapter delegate consent and acceptance form   34 KB   1 version

This form can be used as is, or as a template for creating a similar form for your Chapter. The Delegate Consent and Acceptance Form can be used by any chapter as a means to: 1) collect important delegate biographical information, 2) collect relevant medical and insurance information, 3) to obtain consent for medical treatment, 4) to obtain indemnification and release of liability against claims, 5) to obtain verification that the delegate understands the organizers’ rules and regulations, 6) to obtain a release to use photos and videos, and 7) to verify that the delegate understands the Youth Activity Safety Policy. The organizing Chapter should fill in the legal name of the event in all (Insertion points) within the document. We do not necessarily recommend that you refuse admission to your event if a delegate refuses to provide their insurance information. While it is to the delegate’s benefit and the organizer’s benefit to have this information—as it makes providing speedy care easier—it is difficult to compel this information.

This document is a template intended to work in conjunction with the Delegate Consent and Acceptance Form. We recommend publishing only the Delegate Consent and Acceptance Form with your registration materials—and not publishing this form. While we discourage delegates from refusing care, it is their right to do so. Should a delegate refuse to sign the Delegate Consent and Acceptance Form, we recommend providing this form—directly to the delegate—as an alternative. This form is identical to the Delegate Consent and Acceptance Form in all ways, except the Consent/Refusal. It is also recommended that organizers consider requiring a parent or authorized care giver to accompany any delegate, who has refused to consent to medical treatment, to attend the event.

If your chapter has decided to utilize a third party provider, such as Trak-1, to conduct background checks for adults (teaching artists, chaperones, vendors, exhibitors, et al) attending your festival or conference, you will need each individual to sign a consent for this background check. This is a requirement any time you are outsourcing background checks to a consumer reporting agency. If you are conducting background checks yourself, directly through the National Sex Offender Registry (https://www.nsopw.gov/), you do not need consent. Because background checks may access personal, non-public, information, we have limited the scope of the report to public record information such as: appearance on the National Sex Offender Registry, criminal records, and other public records.

I was looking for the Request Assistance form. I was also looking for the form that we discussed at Summit that every adult should sign (something having to do with them signing off that they are not a sex offender).
