When I taught high school theatre, I used The Stage and the School. Since leaving to teach in a college theatre education program, I discovered Theatre: Art in Action which I really like better; it has hands-on, interactive activities. It covers all areas of theatre. It's an award winning book. Hard copies through MacGraw Hill are expensive, but they have online student texts:

I also bought about 12 used copies from a Google search for under $5.00 each, and some for under $2 if you want hard copies.

Drama Projects, Perfection Learning (This textbook comes with a TON of teacher resources that includes worksheets and templates).
Introduction to Theatre 1 & 2, Suzi Zimmerman (This is more of a workbook and has a lot of great activities for students).
