I have had great success with Michael Rohd's:" theatre for community conflict and dialogue".  He has lots of activities for building ensemble, including some classic trust exercises as well as lots to say about forum theatre.

I highly recommend getting Augusto Boal's "Games for Actors and Non-Actors", and reading his "Theatre of the Oppressed" if you have not done so already. My favorite trust exercises he wrote about in his "Games" book are "Unsighted Cars" and "Hand Hypnosis".
