In the past we did a set up where the knife thrower has a basket next to him and he puts the knifes in the basket and knife handles with pins on them are punched through from the back to pop balloons that where covering the holes.
It takes practice to get the timing right but the ballon popping and the sound of the knifes being smacked into place will draw the audiences attention away from the knife thrower.
