For those who may be doing playwriting units or encouraging students to submit to competitions (like YouthPLAYS' New Voices competition!), the Dramatists Guild now has their formats for plays and musicals online here for everyone:

I strongly recommend using these (my rec is DG Modern for non-musicals).
