I use the attached sheet for each Thespian and keep it in a three ring binder. Once or twice a year I have each Thespian (or potential Thespian) fill out a point assessment form I created which I also keep. I use that assessment sheet to determine the points earned by each student. It asks for information for the most common ways to earn points such as acting, tech work, etc. There is also another question where it asks students to list any other theatrical activity on the back so I do not miss any less common way a student may have earned points during the assessment period.
I have found this system works very well and have been doing it this way for years.
